The purpose of the NSW dermatology form is to support trained pharmacists to treat the following conditions:
- Mild to moderate atopic dermatitis, 6 months to 65 years old
- Herpes Zoster (shingles), 18 years and older
- Impetigo, 12 months or above
- Mild plaque psoriasis, 18 years and older

Please follow these steps:
- In the left-hand menu select Patients in the left-hand menu.
- Search for the patient by surname, first name, address, medicare number or date of birth OR select from the Recently dispensed customers list
Please note: If the patient is not visible select Add patient and manually enter all compulsory fields, including name, address and mobile number.
- Next to the patient's details, select Record Service
- View the Patient Information, including any patients notes, consultation history, medication list and more. Select the Services drop-down
- Select Dermatology NSW
- Review and update the Patient information including all compulsory fields e.g e-mail, date of birth, gender and main language spoken at home etc
Please note: DVA and Medicare number and expiry are NOT compulsory fields. Please enter valid medicare card details OR leave these blank if the patient does not have a valid medicare or DVA number.
Pharmacist details
- Today's date is prepopulated. Select from the Pharmacist dropdown under Pharmacist details
- Select the pencil icon to update your details
Please note: If you do not see yourself in the drop-down list, click the + icon to add your details.
- Indvidual identifier (HPI-I) and Organisation identifier (HPI-O) must be entered for Dermatology. These details can be found in PRODA and in your dispense system. Please check with your dispense software vendor on where this is located.
- Leave the Prescriber No. and eRX Registration No. sections blank
Please note: eRX cannot be utilised during the Trial because a Clinical Trial Treatment Summary will be generated, not a prescription
- Enter Work email
- Select Update details
Age and prior diagnosis verification
- Select the Age verification acknowledgement checkbox
Please note: We recommend to update patient information in your dispense software.
- Answer the below question
Please note: If No is selected the patient can be marked as ineligbile by selecting Cancel Service
Consent
- PATIENT CONSENT- The patient must consent to the service and financial consent
Medical and social history
As long as all required fields are filled out, the Medical and social history section will expand.
- Enter Past medical history and Past surgical history
- Review Medications list from dispense
- Fill out Current medications and Allergies and known adverse medication reactions
- Select the Smoking history (including vaping) drop-down and choose from the options
- Select the Alcohol history drop-down and choose from the options. Enter the amount under Standard drinks per day on a typical drinking day
- Complete Recreation drugs, Relevant family history, Relevant sexual activity and Relevant work, hobbies and other information sections (where applicable)
- Complete the Risk factors for referral or treatment questions
Clinical service
- Under Presenting symptom (up to four), start typing the symptom and select from the list.
Please note: At least 1 symptom must be selected.
- In the text box under Subjective assessment, type the details of presenting symptom or complaint. This field is mandatory.
- In the text box under Objective assessment, type the details of the physical examination and/or other objective assessments. Remember to address risk factors marked as "Yes" on the previous section. This field is mandatory.
- Enter Diagnosis and Treatment and plan. Both of these fields are mandatory.
- Select the prescribed medication(s)
- Upload any supporting files and images by selecting the Upload files button to attach files or images from your desktop computer.
Notifications and referrals (powered by FOXO)
Please note: DO NOT go to Foxo directly to create an account. Your account will be created as part of the MedAdvisor onboarding process.

If a patient has a regular GP:
- any communication (notification or referral) must be sent via secure message.
- If a referral is required a notification does not need to be sent.
- If a referral is not being sent to the regular GP a notification is mandatory.
- If a referral is being sent to an alternate health practitioner or emergency department a GP notification is required.

If patient does not have a regular GP:
- notification is not required
- If a referral is required, it can be sent via secure message OR PDF as applicable.
Please note: All communications require patient consent. If the patient does not consent the form can be completed without comms.
To send secure messages, please follow these steps:
- In the forms Notifications and referrals section, select the drop down under Who do you want to notify / refer to?
- Select the type of communication
If you choose Notify via secure message or Referral via secure message, you will need to make sure you have a valid email address in your pharmacist settings.
- Select Find a health practitioner to search
- The Search contacts pop-up will appear. Enter name, search by discipline, clinic, suburb or state and click Search to locate a health practitioner in the Suggested section.
- Select the Health Practitioner you would like to securely message from the list OR select Add new contact if you cannot find them in the search results.
- The template for Subject and Body of the notifcation will pre-populate. You can edit these fields as required.
Please note: When generating a referral, be sure to edit this text by entering specific reasons for the referral.
Referral reasons include:
| Referred to a GP as required by clinical guidelines 'red flag' or 'refer when' criteria |
| Referred to an Emergency Department as required by clinical guidelines 'red flag' or 'refer when' criteria |
| Patient has a condition outside the scope of the trial |
| Requires further investigation or tests, including pathology |
- Select Preview message to view the message with all fields (e.g Patient info) visible.
- All consultation details including uploaded supporting files and images will be attached to the message as a PDF by default. This can be deselected.
- After reviewing / editing the message, select Send secure message
- A green bar will appear, letting you know the message was sent successfully.
- You will also see a record below the message, showing you the date and time of the successfully sent message.
- Repeat the above steps by selecting + Add another communication to message another health practitioner, as required.
You must set up your pharmacist profile, prior to using Notifcation and referrals to send secure messages via Foxo. More information can be found here: https://foxo.com/
Consultation outcome
- Select the Consultation length drop-down and select the length
- Select a dermatological condition from the Clinical Service Type drop-down list.
- Click REVIEW CLINICAL GUIDELINES to review the protocols.
- Answer the compulsory question, Does the patient have a regular GP?
- Answer the mandatory questions, Is a referral required? and Does the patient consent to the pharmacist sending a notification or referral, as applicable?
Please note: If the patient consents and a notification or referral is required, scroll up to Notification and referrals to complete the mandatory notification or referral.
- Select from the list in Consultation summary (At least one option must be marked)
Please note: If No intervention is selected, all other fields will be greyed out and cannot be selected.
- Selecting Referral made requires you to select a reason(s) for the referral
At the bottom (and the top) of the form, you can Save a draft at any time. You can also Delete the form or Complete the form, as long as you have completed all compulsory fields.
