The Medadvisor for Pharmacy OCP Initiation NSW form supports the provision of the service under the funded pilot, or as a private service for patients who do not want to participate in the pilot evaluation.
Please follow these steps:
- In the Medadvisor for Pharmacy menu, select Patients
- Search for the patient, select from the recently dispensed list or select +Add patient
- Review the patient information including notes, Clinical Summary, Care Opportunities and Medication list. Select the Services drop down
- Select OCP Initiation NSW
- Review the Patient Information and fill out all compulsory fields e.g DOB and Sex
Please note: We recommend to confirm and update the patients' email address to ensure you can send the patient an e-mail at the completion of the service
- In the Consulting pharmacist details section, today's date is pre-populated but can be changed. Select your profile from the Pharmacist drop-down. If you are new to the pharmacy, select the + icon to add your details.
- Under Pharmacist suitability, select the checkbox to confirm you have completed the required training and will explain the purpose of the patient questionnaire and program
- Select the checkbox after you have informed the patient and receive consent
- Answer the Pilot participation question and upload paper screening questionnaires (where applicable) by selecting Upload files
- If Yes - funded pilot inclusion is selected, tick the checkbox to ensure the patient consents to complete the screening questionnaire and has provided informed pilot participation consent
- Complete the Screening questionnaire (digital record), if you have not uploaded the paper version
Please note: The screening questionnaire must be completed for funded pilot services. If completed on paper, a copy can be uploaded and stored with the consultation form. An optional digital record can be entered.

- Complete the Clinical assessment
- Complete the Treatment section, ensuring Service type is selected
- Select the appropriate boxes for items in the Non-pharmacological & women's health advice section
An email can be sent to the patient with links to helpful resources. Tick the box in the Non-pharmcological & women's health advice section and ensure the patients' correct email address is entered to automatically send when the form is completed.
- Send a notification and/or referral to the patients GP or medical provider where appropriate. For more on Notifications and referrals, including secure messaging CLICK HERE
- Once all compulsory fields are completed, you can select Complete or Save to save a draft and come back to it.
The record will be saved in the patient profile and will be shown in Recent Services.
