Applies to: MedAdvisor App, PlusOne Calendar, PlusOne Record Services, COVID-19 Pre-Screening Form.
Customers are able to complete a pre-screening consent form for COVID vaccination bookings via the Professional Services section of the MedAdvisor App and / or the customer booking link provided to customers via a PlusOne Calendar link.
The pre-screening form is automatically turned on for all pharmacies, however, filling out the form is optional for customers.
Please note, the pre-screening form has to be done within 24 hours. In the case where an appointment has been booked outside of 24 hours e.g. booked today for next week, the pre-screening form would come with the reminder (email and / or SMS depending on Calendar Settings) 24 hours prior.
If a customer chooses to fill out the pre-screening form before their appointment on the App or Web Link, below is the information they are required to fill out:
1. First Name, Last Name, Mobile, Medicare Number, DOB, Address and Email (and security question)
2. If a booking is made less than 24 hours prior to the appointment, the pre-screening consent form can be completed once submit is selected.
The customer can tick if any of the below is applicable to them and can write any additional notes for the pharmacy, then Confirm Booking and the patient will receive a booking confirmation.
Note: If the patient ticks the box above the patient will be sent a link to sign up to the app and view vaccination history in 'track my results' COVID Vaccinations - Track my Results
If any fields are missed, a pop up is displayed notifying the patient that they have missed a question
NEW UPDATE: A new update to the pre-screen form, it now contains two additional questions which the patient is required to answer.
3. UPDATE The COVID vaccination pre-screening form asks the patient to give consent by ticking the box for 'Patient has consented to vaccination' and 'Patient is assessed as suitable for vaccination' – if these details are not filled out, a pop up box will appear to ask the patient to complete.
Important: The patient fills out the details in the pre-screen form and then confirms that that they have consented to the vaccination. The Pharmacy also needs to confirm by ticking both boxes (above) that the patient has consented and is assessed as suitable for the vaccination (this is mandatory).
How to access the completed pre-screening form in PlusOne
When the customer comes in to the pharmacy at their booked time, the pharmacy can access their pre-screening form via the following 2 options:
OPTION 1. Health Services Hub > COVID 19 tile > Print Completed Pre - Screening form
The pre-screening form will have the First Name, Last Name, Mobile, Email, Medicare, DOB, address and any pre-screen conditions ticked with notes.
OPTION 2: PlusOne Calendar > Click on appointment > Select Print Pre-Screening Form or 'Open in Record Services'.
If the 'Open in Record Services' option is selected the pre-screen form will populate in the patients profile in Record Services.
The patients info, the ticked pre-screen form including YES AND NO and notes, also if the patient ticked that they would like to see their history (this box will be ticked at the bottom of the screen) and all information will be visible in the pre-screening checklist section.
